Module 3 · Data flows in a national HIS
A national health information system is a chain of custody for data. In this module we follow a single antenatal care visit in Kinshasa from the paper register, through the facility EMR, into the district DHIS2 instance and finally onto the national dashboard used by the Ministry of Health. At each hop we ask three questions: who owns the record, what can be lost, and what latency is acceptable.
- Trace a single clinical record from point of care to national dashboard.
- Identify where data loss and reporting delay occur in a district chain.
- Explain the ownership and stewardship model at each layer.
- Judge which exchange standard fits a given integration.
DHIS2 District Health Information Software, the aggregate reporting platform used across most African health ministries.
EMR Electronic Medical Record, the facility level clinical system.
Data latency The delay between a clinical event happening and it becoming visible on a national dashboard.
Which standard would you adopt to exchange a clinical record between an EMR and a national shared health record?
HL7 FHIR is the right answer. It defines resource level exchange over a REST API, which is exactly what a national shared health record needs.
